Ann Arbor Endocrinology and Diabetes Associates (AAEDA) has made the following pages available for quick download and printing so that our patients can conveniently complete and bring to the office on their first visit.

Administrative
New patients to the offices of AAEDA will need to print, complete, and sign both the New Patient Information Form, as well as the signature page of the HIPAA Compliance form. Please bring the forms with you to your first visit. Please also complete the Patient Information Form if any of your contact or insurance information has recently changed.

AAEDA now accepts Visa, Mastercard and Discover as payment at time of service.

Our office has a policy that all patients leaving routine messages are called back within 24 to 48 hours. If for some reason you do not receive a callback within two days, please call again as we may not have received your message clearly.
